As the NHS has set out its 10 year health plan, the role of primary care has never been more central to the future of the health and care system. At CRGPA, we welcome the plan’s ambition to deliver more personalised, proactive and community based care, and we are proud that so much of our work already aligns with these priorities.
Our mission has always been clear: to support practices, strengthen resilience, and improve outcomes for the communities we serve. The 10 year plan reinforces the importance of this work, and we are well positioned to help turn its aspirations into meaningful, measurable progress across Coventry and Rugby.
Delivering care closer to home
The plan places a strong emphasis on shifting care into the community and reducing avoidable hospital admissions. CRGPA’s services already demonstrate the impact of this approach.
Between October & December 2025 alone;
- Our Paramedics Acute Visiting Service team visited 2,657 patients in their homes between October and December 2025, helping 87% avoid hospital admission.
- The Respiratory@Home service supported 623 people with acute respiratory infections, providing rapid assessment and treatment in the place patients feel safest.
- Our work with frail and housebound patients continues to ensure that those with the greatest needs receive timely, compassionate and clinically robust care.
These services reflect the very principles the 10‑Year Plan champions: early intervention, integrated support, and care delivered where it makes the biggest difference.
Supporting prevention and long term condition management
The plan highlights prevention and proactive management of long‑term conditions as essential to reducing pressure on urgent and emergency care. CRGPA is already contributing to this shift.
In the last quarter alone, 835 people with severe mental illness received an annual health check through our teams. This work not only improves health outcomes but also strengthens continuity of care and early identification of risk.
Building a resilient, empowered primary care workforce
A sustainable NHS depends on a strong, supported workforce. CRGPA continues to invest in leadership, operational clarity and clinical excellence across our services.
Our Chief Nurse and Operations Officer, Sarah Forsyth, has led significant improvements in service performance, including increasing utilisation of the Paramedic Acute Visting Service from 75% to over 90%. Her focus on empowering teams, strengthening processes and ensuring high‑quality care mirrors the workforce ambitions set out in the 10‑Year Plan.
Strengthening collaboration across the system
The 10‑Year Plan calls for deeper collaboration across primary, community and acute care. CRGPA is committed to this shared responsibility.
We continue to work closely with practices to understand variation, share best practice and ensure equitable access to services. Our engagement with system partners helps us navigate challenges such as community pathway pressures and referral complexity, ensuring that patient care remains safe, coordinated and effective.
Looking ahead
As we move into 2026, CRGPA is preparing for the next phase of development. The Practice Director nominations went live in February, ensuring our new leadership is in place and ready to help guide our strategic direction for the upcoming financial year. We are also launching a new Shareholder Zone, designed to enhance transparency, strengthen communication, and support shared learning across our member practices.
The NHS 10‑Year Plan sets out an ambitious vision for the future. CRGPA is ready to play a leading role in delivering it strengthening primary care, supporting our workforce, and improving outcomes for the communities we serve.
