Respiratory@Home

Our Respiratory@Home service supports and monitors patients with respiratory conditions while they remain in the comfort of their own homes. Our virtual ward offers an alternative pathway to hospital admission and provides a safe discharge option for patients who still need ongoing monitoring.

The process is simple and easy to follow. Once we receive a referral, we contact the patient directly to onboard them into the service.  If clinically appropriate and needed we will send a pulse oximeter and an information pack in the post, explaining how to record and monitor their oxygen levels. This allows our team of care coordinators and GPs to have an overview of  their readings and provide support throughout their time with us.

Referral criteria

Adults (18 years and over) with confirmed or suspected acute respiratory illness, including COVID, who are stable or improving but require ongoing monitoring that can be safely provided in their home or usual place of residence.

  • Flu
  • RSV
  • COVID
  • COPD exacerbation
  • Asthma exacerbation

The referral process

Referrals are accepted from the following:

  • GPs
  • Self-Referrals
  • Emergency Departments
  • Ambulance/Maternity services
  • 111
  • Hospital respiratory wards
