Severe mental illness annual health checks
An SMI (Severe Mental Illness) physical health check is a free, annual and comprehensive assessment for adults (18+) with a diagnosis of schizophrenia, psychosis or bipolar disorder. We have a team of experienced healthcare assistants who carry out these checks on behalf of GP Practices across Coventry and Warwickshire. SMI patients are contacted to make an appointment, either at their GP Practice or an alternative venue and arrange an appointment to carry out the following as part of the check:

Referral criteria
Adults (18 years and over) who have a confirmed diagnosis of schizophrenia, bipolar, obsessive compulsive, post traumatic or psychosis and are on the practices’ mental health register.
Referral process
Any practice which signs up to the service, is contacted by the GPA to either submit their SMI registers or agree to the use of EMIS search and report. You can contact The SMI service team via:
